Healthcare Provider Referral

Referrals are welcomed at EyeHaus for specialty services such as scleral lens fitting, orthokeratology fitting, myopia management, urgent care, and ZEST treatment. We believe collaborations between providers is essential for continuity of care. The patient will return to the referring provider after the specialty service unless otherwise indicated. Please expect a response letter once we have seen the patient.
